Inhaler therapy plays a key role in the management of respiratory conditions such as asthma and COPD, but treatment effectiveness depends greatly on selecting the appropriate device and using the correct technique. This session will provide a practical overview of different inhaler devices, their clinical indications, and how to choose the right device for individual patients. The discussion will also highlight common technique errors, patient education, and strategies to improve adherence and treatment outcomes.

  • Inhaled medication is a cornerstone of pulmonary therapy, particularly for asthma, chronic obstructive pulmonary disease, and cystic fibrosis. By delivering drugs directly to the airways, clinicians achieve high local concentrations with significantly reduced systemic side effects compared to oral or intravenous routes. The therapeutic efficacy of aerosolized therapy is primarily dictated by particle size; a mass median aerodynamic diameter of two to five micrometers is optimal for deposition in the bronchi and bronchioles. Particles larger than five micrometers typically result in oropharyngeal deposition and drug wastage, while those smaller than one micrometer often remain suspended and are exhaled. Extra-fine particle formulations, often utilizing hydrofluoroalkane propellants, have been developed to enhance peripheral lung delivery, though clinicians must monitor for increased systemic absorption due to the high surface area of the alveolar region.
  • Selection of the appropriate delivery device is critical and must be tailored to the patient. Common interfaces include pressurized metered-dose inhalers, dry powder inhalers, and nebulizers. Pressurized metered-dose inhalers are portable and cost-effective but require high levels of hand-breath coordination, which can be mitigated by the use of spacers or breath-actuated devices. Dry powder inhalers are breath-actuated and eliminate the need for coordination, yet they necessitate a sufficient inspiratory flow rate, making them unsuitable for very young children or those with significant respiratory muscle weakness. Nebulizers remain essential for patients unable to coordinate inhalation, such as those in acute distress, pediatric or elderly patients, and those requiring mechanical ventilation.
  • Beyond standard bronchodilators and corticosteroids, inhaled therapy has expanded to include antibiotics for chronic infections, such as those caused by Pseudomonas in cystic fibrosis, and vasodilators for pulmonary hypertension. Emerging research is currently investigating inhaled anti-fibrotic agents and biologics for severe, steroid-resistant disease. Regardless of the device or medication chosen, treatment success depends on the patient's adherence and technique. Clinicians must perform regular assessments of inhaler technique, especially when a patient’s condition appears to be worsening, to ensure that therapeutic failure is not a result of improper device use rather than disease progression.
