Postmenopausal bleeding (PMB) is defined as any vaginal bleeding occurring after 12 months of amenorrhea in a woman of menopausal age and is considered an important clinical concern. Though often caused by benign conditions such as endometrial atrophy, polyps, or hormone replacement therapy, PMB may also signal more serious issues like endometrial hyperplasia or malignancy. A thorough clinical evaluation—including transvaginal ultrasound and, when necessary, endometrial biopsy—is essential for timely diagnosis and management. This case discussion focuses on differential diagnosis, risk stratification, and evidence-based pathways for investigation and treatment. Early intervention can significantly impact patient outcomes, especially in detecting endometrial cancer at a treatable stage.

Dr. Sumana Talakokkula is a seasoned Consultant Gynecologist and Obstetrician based in Hyderabad, with over 22 years of experience in women’s health . She earned her MBBS from Gandhi Medical College, followed by specialist diplomas in Obstetrics & Gynecology and Anesthesiology from Osmania Medical College and the College of Physicians & Surgeons, Mumbai. Dr. Talakokkula has held key positions, including Consultant Gynecologist at Maheshwara Medical College & Hospital (2015–2017), and currently practices at leading clinics such as Apollo and SLG Hospitals. Her expertise spans a broad range of services—PCOS management, fertility treatments, laparoscopic surgery, hysterectomy, prenatal care, and endoscopy among others. A member of FOGSI and IMA, she is also actively involved in academic medicine, having presented research at state and national conferences
