This 60 minutes live webinar will cover “Role of dietary led behavioral interventions In obesity Management”.Dietary modification is central to obesity treatment. Weight loss diets are available that include various permutations of energy restriction, macronutrients, foods, and dietary intake patterns. Caloric restriction is the common pathway for weight reduction, but different diets may induce weight loss by varied additional mechanisms, including by facilitating dietary adherence. This narrative Review of meta-analyses and select clinical trials found that lower-calorie diets, compared with higher-calorie regimens, reliably induced larger short-term (12 months). Few significant long-term differences in weight loss were observed for diets of varying macronutrient composition, although some regimens were found to have short-term advantages (e.g., low carbohydrate versus low fat). Progress in improving dietary adherence, which is critical to both short- and long-term weight loss, could result from greater efforts to identify behavioral and metabolic phenotypes among dieters.

ملخص

  • Ma, a clinical dietician with 11 years of experience in Dubai, works at Medic Clinic, a leading private healthcare provider. She coordinates an obesity program certified by the European Society for the Study of Obesity (EASO) as a center of excellence, involving a multi-disciplinary team.
  • The program emphasizes a multi-disciplinary approach, with dieticians, endocrinologists, psychologists, bariatric surgeons, and gastroenterologists collaborating on complex obesity cases. These monthly meetings ensure a comprehensive and tailored approach for each patient.
  • Recent international guidelines highlight the importance of behavioral interventions in obesity treatment. The Canadian guidelines specifically mention developing confidence, overcoming barriers, and increasing intrinsic motivation, treating obesity as a chronic condition rather than a lack of willpower.
  • The program includes virtual support groups for patients, covering obesity causes, management, treatment modalities, and challenges. These sessions feature testimonials from successful patients to provide motivation and a patient perspective.
  • Ma has pursued additional training in Cognitive Behavioral Therapy (CBT) to enhance her dietician skills with behavioral aspects. While not replacing a psychologist, incorporating motivational interventions can improve treatment adherence and facilitate referrals when needed.
  • A successful weight management plan integrates sleep management, stress management, medical, bariatric, nutritional, and physical interventions, all supported by psychological and behavioral aspects. It is crucial to incorporate these components into a balanced strategy.
  • Dieticians need to incorporate realistic goal setting, motivational interviewing, building trust, shared decision-making, self-monitoring, and managing food cravings. The process empowers patients, fostering resilience and addressing internalized weight bias.
  • Motivational interviews focus on support, partnership, exploring patient concerns, and provoking the individual's reasons for change. Patients are empowered as drivers of their own transformation, taking accountability for their health goals.
  • Shared decision-making involves balanced communication, considering benefits, risks, values, and preferences. It ensures patient satisfaction, reduces provider burnout, increases follow-up rates, and improves overall communication and outcomes.
  • Self-monitoring, including tracking food intake and exercise, enhances accountability and increases weight loss percentages, which is supported by research. Combining intensive counseling with smartphone apps has shown to have a positive impact.
  • Weight bias is a negative stereotype attributing obesity to personal choice, which can lower self-esteem and increase internalized bias. It's crucial to reframe obesity as a chronic, progressive medical condition with genetic and environmental influences.
  • Unrealistic expectations should be addressed by focusing on a healthy body weight, achieved through sustainable lifestyle changes while still enjoying life. The goal is to correlate weight loss with improved health benefits and quality of life.
  • Caloric deficit, not carbohydrate intake, is the primary driver of body fat loss. Adherence to a diet, rather than a specific type of diet, is essential for success. Tracking and self-monitoring are vital for effective weight management.
  • Clarifying values and motivations is key to increasing commitment. Patients should reflect on whether their daily decisions align with their values, promoting intrinsic motivation and willingness to sustain long-term efforts.
  • Understanding and addressing "wanting," the subconscious motivational force driving overeating, is essential. By mapping out triggers and finding distractive strategies, patients can reduce the desire for unhealthy foods.
  • Restraint, the capacity to say no to cravings, requires willpower and cognitive effort. Cognitive Behavioral Therapy (CBT) strengthens rational decision-making, helping patients make better food choices.
  • Sleep, caffeine, stress, and depression significantly impact weight management. It's important to emphasize daily physical activity, good sleep patterns, and stress management techniques.
  • Resilience, the ability to respond positively to setbacks, can be learned and improved. Overcoming negative thoughts and embracing current resources and skills are crucial for long-term success.
  • Long-term weight loss success is associated with specific behaviors: low-calorie, low-fat diet, breakfast consumption, weekly weight checks, limited TV viewing, and regular exercise. Mimicking these actions can improve patient outcomes.
  • Successful weight management involves listening to the patient, guiding them toward their goals, offering information, and considering complementary options. A multi-disciplinary approach, involving a dedicated team, is essential for addressing the complexities of obesity.
