The clinical rotation in the US introduces international students to American Medicine as they would experience it in a hospital-based residency programme. It also familiarizes and acclimates an international medical graduate to the practice of medicine in an American clinical setting.

- The USMLE journey consists of four parts: Step 1, Step 2 CK, OET, and Step 3. Completing Step 1, having a medical degree, OET certification, and Step 2 CK allows one to become ECFMG certified and apply for residency programs. Letters of recommendation (LORs) are crucial for residency applications, with US-based hospital LORs carrying more weight.
- Clinical rotations are essential for US residency applications. Observerships or shadowing are for medical students, while electives or externships, which involve more hands-on experience, are generally for medical graduates. These rotations are necessary to obtain the required LORs for residency application and to build a strong resume.
- The Step 1 exam has changed from a three-digit score to a pass/fail system. While removing the pressure of a numerical score, it increases the importance of clinical rotations and resume building. Tele rotations can also be valuable if done through well-known institutions.
- The US clinical setting is different from those in other countries. The weightage of LORs from academically inclined physicians is significant and much more valuable than generic LORs from big universities or private practices. Choosing rotations wisely is crucial for building a strong residency application.
- After rotations and residency applications, candidates will go through the interview cycle. Residency interviews have become more virtual, calling more candidates. Therefore, doing clinical rotations and showing what one has done helps a lot. Physicians are often involved in research projects.
