Description

Pneumonia is a respiratory infection that inflames the air sacs in one or both lungs, causing symptoms such as cough, fever, shortness of breath, and chest pain. Diagnosing pneumonia typically involves a physical exam, where doctors listen for abnormal lung sounds, followed by imaging tests like chest X-rays to confirm lung involvement. Laboratory tests, including blood tests and sputum cultures, help identify the causative agent, whether bacterial, viral, or fungal.

  • Dr. Bal Muran discusses pneumonia diagnosis and treatment. The patient presented with shortness of breath, right-sided chest pain, and fever with chills and rigors. There was no V or hemoptysis. The patient is a chronic smoker with a significant pack-year history and multiple unsuccessful attempts to quit. He is also a known diabetic on insulin and oral hypoglycemic agents, and has a history of tuberculosis cured six months prior, and hypertension.
  • The patient was taking human insulin, glucon, telisa, and amod for his pre-existing conditions. For the presenting symptoms, he was prescribed amoxicillin and syrup asoral. The speaker notes that the amoxicillin dosage was incorrect and that a cough suppressant should not be given for a patient with expectoration; instead, an expectorant and mucolytic would be more appropriate.
  • Physical examination revealed the patient was febrile, tachypneic, and tachycardic, maintaining saturation on room air. Respiratory examination showed diminished movements of the right hemithorax with increased vocal resonance and fremitus, tubular bronchial breath sounds, and crepitations.
  • Based on the symptoms and examination findings, the probable diagnosis was right upper lobe pneumonia. Pneumonia is defined as an infection of the pulmonary parenchyma with consolidation, essentially a disease of the lungs involving the distal air space.
